Get ready for MS-DRGs: Evaluate current coding and documentation practices

The key to success under any severity-adjusted DRG system is complete and accurate documentation, coupled with complete and accurate coding. Start by asking the question, how accurate is my coding today? Organizations can use historical data and chart reviews to evaluate current coding and documentation practices. As the CC/MCC list is very different in the MS-DRGs system than in V24, here are some areas where organizations should focus their efforts: - Some studies have shown a correlation between the number of secondary diagnosis codes and severity level. Use historical data to measure just how many secondary diagnoses are being coded.
- Perform a chart review, selecting charts either at random or with a specific, targeted focus, depending on the organization’s objectives.
- Evaluate coding thoroughness—What isn’t being coded that might be?
- Evaluate coding accuracy—Get credit for the level of care you deliver.
- Evaluate your current approach to getting MD documentation. Try to identify areas for improvement.
