The E/M codes were designed to increase accuracy and consistency of reporting levels of cognitive encounters with patients. The definitions of E/M codes are based upon the degree of work and difficulty of diagnosis and treatment. Although E/M codes are frequently used, they can be interpreted many ways and they are still misunderstood.

Evaluation & Management Coding: The Use of CPT^® E/M Codes is a training tool that guides physicians in selecting the correct code based on work, and helps staff and compliance personnel ensure that their medical record documentation substantiates the level of E/M service code selected.
